Provo Peaks Elementary School

We’re excited to announce that Provo City School District is offering evening hours for school registration this summer! These extended hours are designed to make the registration process as convenient as possible for busy families. Parents and legal guardians are invited to register all their children in one location, during one visit.

Children are welcome to attend, but their presence is not required.

Registration Dates, Times, and Locations

Choose the date and location that works best for you:

  • July 16 & 17
    • Provo High School — 1199 Lakeshore Dr
    • 3:00 PM – 6:00 PM
  • July 22
    • Centennial Middle School — 305 E 2320 N
    • 3:00 PM – 6:00 PM
  • August 6 & 7
    • Provo City School District (PDC) — 280 W 940 N
    • 3:00 PM – 7:00 PM

A parent or legal guardian must be present to complete registration.

What to Bring for New Student Registration

If you’re registering a new student, please come prepared with the following documentation:

  1. Proof of Age (Choose ONE of the following)
    • Original long-form birth certificate
    • Passport with a notarized affidavit
    • Reliable legal document with a notarized affidavit
    • A notary will be on-site. Be sure to bring a government-issued photo ID for notarization.
  2. Immunization Records
    • Official immunization record OR
    • Valid exemption certificate
    • Note: A TB test may be required, which can be done through your doctor or the Utah County Health Department.
  3. Proof of Address (Must include parent’s name and current address)
    • Acceptable documents include:
      • Copy of deed and/or most recent mortgage payment
      • Lease and rent receipt or proof of payment
      • Legal affidavit from landlord confirming tenancy
      • Utility bills (gas, electricity, internet, cable)
      • Letter from a government agency dated within the last 60 days
      • Payroll stubs
      • Vehicle registration
      • Religious or church records with an affidavit from a recognized leader
      • W-2 form (dated within the past year)
      • Property tax bill
    • NOT Accepted as Proof of Address:
      • Bank statements, medical mail, advertisements, or personal mail
  4. If You’re Living With Another Family, Provide a “Living with Another Family” Affidavit.
    • Both the parent and the homeowner must:
      • Be present
      • Bring a government-issued photo ID
      • Bring valid proof of address for the homeowner
  5. Parent’s Photo ID
    • Accepted forms of ID:
      • Driver’s license
      • State-issued ID
      • U.S. or foreign passport
      • U.S. Military ID
      • Permanent Resident Alien Card
      • Other current government-issued ID
      • ID must be current, not expired, and in a language the on-site notary can understand.
